Notification of a Deceased Member

If a member of your household has died please notify HealthSource RI.

The following documents are accepted as proof of death:

• Copy of death certificate
• Copy of death notice from newspaper, or
• Copy of other official notice of death

If there is an incorrect request asking to verify a deceased status, the individual may come to the Walk-In Center with a Photo ID. If you have already taken this action, please contact HSRI to explain in order for us to escalate the concern appropriately. We apologize for this inconvenience.
