Submitting Documents to Confirm Your Eligibility
How to Submit Documentation:
- In Person: Visit the Contact Center located at 401 Wampanoag Trail, East Providence, RI 02915
- Online: Go to your HealthyRhode customer portal at HealthyRhode.ri.gov and upload the documents

These documents are helpful to have if you’re signing up for health insurance:
- Photo ID of the primary applicant (driver’s license or passport)
- Social Security Numbers for everyone applying
- Information on current health insurance (if any)
- Names of any doctors you want to keep
- Current monthly and projected annual income
- Proof of immigration status if not a U.S. citizen
*Note: Income information is necessary if you want to see if you qualify for financial help. You will need to provide accurate income for what you earn monthly and for the entire year. If documents will help with this, you can bring those.
